Job summary

Optimal Care is seeking a Nurse Practitioner or Physician Assistant to deliver primary and consultative care to geriatric and medically complex patients, often in their homes or care settings. You will develop and manage personalized treatment plans, prescribe medications, and collaborate with physicians and care teams to improve comfort and quality of life.

Qualifications

• Nursing skills as defined by accepted nursing standards.
• Ability to assess and respond to the needs of patients, families and caregivers in post-acute care settings.
• Ability to handle multiple priorities, documentation requirements and deadlines.

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients, often in homes or care settings.
• Develop and manage individualized treatment plans, prescribe and monitor medications.
• Coordinate with physicians, caregivers, and interdisciplinary team members within a collaborative practice.
