Job summary

Talin Home Healthcare LLC in Sterling, VA seeks compassionate Home Health Nurses to serve patients across Loudoun County and the NOVA region. You will deliver high-quality, personalized in-home care, build meaningful connections with patients, and thrive in an independent work environment.

Qualifications

• Active RN license in Virginia.
• Experience in home health, med-surg, or community nursing preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Willing to travel throughout the NOVA region with reliable transportation.
• Commit to minimum of 2 days per week, Monday plus either Wednesday or Friday.

Responsibilities

• Provide skilled nursing care in patients' homes across the Northern Virginia area.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Must have experience with administering medications, wound care, IV therapy, ostomy care and other clinical treatments as ordered.
• Monitor and document patient progress, vital signs, and changes in condition.
• Educate patients and family members on disease management, medications, and home care techniques.
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.

• Monitor and document patient progress, vital signs, and changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

• Active RN license in the state of Virginia
• Previous home health, hospital med-surg, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for travel throughout the NOVA region
• Ability to work independently and manage a flexible patient caseload
• Compassionate, patient-centered approach to care
• Must commit to working a minimum of 2 days a week 1 day of the days must be a Monday, and the other can be Wednesday or Friday.
