Job summary

Talin Home Healthcare LLC is seeking compassionate Home Health Nurses to serve patients across Loudoun County and the Northern Virginia area from our Sterling, VA base. You will provide high-quality, personalized care in patients' homes, embracing independence and meaningful nurse-patient relationships.

Qualifications

• Active RN license in Virginia.
• Experience in home health, hospital med-surg, or community nursing preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Reliable transportation and valid driver's license for NOVA travel.
• Ability to work independently with a flexible patient caseload.
• Compassionate, patient-centered approach to care.
• Must commit to working a minimum of 2 days per week (Monday plus either Wednesday or Friday).

Responsibilities

• Provide skilled nursing care to patients in their homes across the Northern Virginia area.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Must have experience with administering medications, wound care (wet/dry), wound vac, IV therapy, ostomy care and other clinical treatments as ordered.
• Monitor and document patient progress, vital signs, and changes in condition.
• Educate patients and families on disease management, medications, and home care techniques.
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.

• Ability to work independently and manage a flexible patient caseload
• Compassionate, patient-centered approach to care
• Must commit to working a minimum of 2 days a week 1 day of the days must be a Monday, and the other can be Wednesday or Friday.
