Job summary

Talin Home Healthcare LLC is seeking compassionate Home Health Nurses to serve patients across the Northern Virginia counties area. This role offers independence, meaningful, patient-centered care in patients' homes.

Requirements include an active Virginia RN license, travel throughout the NOVA region, and a commitment to at least 3 days per week (Mon/Wed/Fri). We value a supportive culture and flexible scheduling for our team.

Qualifications

• Active RN license in Virginia.
• Previous home health, med-surg, or community nursing preferred.
• Strong clinical assessment and critical thinking.
• Excellent communication and patient education abilities.
• Willingness to travel throughout NOVA with reliable transportation.
• Commit to at least 3 days per week (Mon/Wed/Fri).

Responsibilities

• Provide skilled nursing care in patients' homes across Northern Virginia.
• Assess patients and develop individualized care plans.
• Administer medications, wound care, IV therapy as ordered.
• Monitor patient progress and document vital signs.
• Educate patients and families on disease management and home care.
• Collaborate with physicians and care team for coordinated care.

About the Role: Talin Home Healthcare LLC is looking for multiple compassionate and dedicated Home Health Nurses to serve patients across different Northern Virginia counties area. Responsibilities:

• Provide skilled nursing care to patients in their homes across the Northern Virginia area
• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered
• Monitor and document patient progress, vital signs, and changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

• Active RN license in the state of Virginia
• Oasis and SOC Experience a preferred and favored
• Previous home health, med-surg, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for travel throughout the NOVA region
• Ability to work independently and manage a flexible patient caseload
• Compassionate, patient-centered approach to care
• Must commit to working a minimum of 3 days a week Mon/Wed/Friday
