Job summary

Talin Home Healthcare LLC in Sterling, VA is seeking multiple compassionate Home Health Nurses to serve patients across the Northern Virginia counties. You will deliver high-quality, personalized care in patients’ homes, thriving in an independent environment and building real connections with families.

Responsibilities include conducting assessments, developing care plans, administering medications, and collaborating with physicians and the care team to ensure coordinated care.

Qualifications

• Active RN license in Virginia is required.
• Experience in home health or med-surg preferred.
• Excellent communication and patient education abilities.
• Reliability for travel across NOVA and flexible caseload.

Responsibilities

• Provide skilled nursing care to patients in their homes across the Northern Virginia area
• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered
• Monitor and document patient progress, vital signs, and changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Responsibilities:

• Provide skilled nursing care to patients in their homes across the Northern Virginia area
• Conduct comprehensive patient assessments and develop individualized care plans

Requirements:

• Active RN license in the state of Virginia
• Previous home health, med-surg, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for travel throughout the NOVA region
• Ability to work independently and manage a flexible patient caseload
• Compassionate, patient-centered approach to care
• Must commit to working a minimum of 2 days a week, with 1 day required to be a Monday or a Tuesday
