Job summary

Freedom Home Health and Hospice Care Services Inc. is seeking a registered nurse to plan, organize and direct home care services with emphasis on community health education.

This on-site role requires a California RN license and CPR certification, with 1–2 years of recent acute care experience preferred. You will assess patients, develop and revise care plans, coordinate with physicians and families, and supervise home health aides as part of the care team.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor’s degree preferred.
• One to two years of recent acute care experience in an institutional setting.
• Management experience not required; responsible for supervising home health aides.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Develops a care plan with goals based on nursing diagnosis and includes patient and family in planning.
• Administers medications and treatments as prescribed.
• Initiates plan of care and revises as patient status changes.
• Acts as Case Manager and coordinates patient care for assigned caseload.
