Job summary

Freedom Home Health and Hospice Care Services Inc. is seeking a registered nurse to plan, organize and direct home care services in the community. The role emphasizes community health education and experience in nursing within patients’ homes.

The RN will assess patients, develop and revise plans of care, coordinate with physicians and families, administer medications, and manage care for an assigned caseload in home settings. Strong communication and problem-solving skills are essential.

Qualifications

• Graduate of an accredited school of nursing.
• Current state licensure and CPR certification.
• Bachelor’s degree with 1 year home health care experience preferred.
• Management experience not required.
• Strong observation, communication, problem solving, and basic math skills per competency checklist.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and includes patient/family in planning.
• Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care instructions per assessment and plan of care.
• Identifies discharge planning needs and coordinates pre-discharge activities.
• Acts as Case Manager and coordinates care for assigned caseload.
