Job summary

Sequoia Home Health and Hospice is seeking a compassionate Registered Nurse (RN) to organize and direct home care services within patients' homes and communities. The RN will develop care plans, provide nursing procedures, and educate patients and families in collaboration with the physician.

Requirements include an active CA RN license, CPR certification, and at least 2 years of nursing experience, preferably in public health or home care.

Qualifications

• Active CA RN license
• Minimum 2 years nursing experience; public health or home care preferred
• CPR certification required
• Must be a licensed driver with insured vehicle
• Strong observation, problem-solving, and communication skills

Responsibilities

• Develop and initiate the plan of care and interdisciplinary assessment with patient and caregiver(s)
• Perform preventative and rehabilitative nursing procedures as ordered
• Provide education to patient, caregiver, and family; counseling as needed
• Prepare clinical notes and coordinate with the physician and other health practitioners
• Identify discharge planning needs and implement before discharge
