Job summary

The Staff Pad is partnering with a mission-driven healthcare organization in Lawrence, MA to hire a Geriatric Nurse Practitioner for its primary care practice. This role offers top-of-license practice with flexible scheduling and meaningful work serving Medicare patients across offices and homes.

As part of a collaborative team, you will conduct Wellness visits, manage chronic conditions, and support advance care planning for older adults.

Qualifications

• Experience caring for geriatric patients.
• Active MA NP/APRN license; ability to obtain NH license.
• Board certification in Adult-Gerontology, Gerontological, or Family NP.

Responsibilities

• Assess, diagnose, and manage acute and chronic conditions in older adults.
• Lead Medicare Annual Wellness Visits and close preventive care gaps.
• Prescribe and reconcile medications, and educate patients and caregivers.
• Provide care management for high-risk and recently hospitalized patients.
• Guide advance care planning and goals-of-care conversations.
• Conduct home and facility visits across office, home, and senior living settings.
