Job summary

OnSite Medical Solutions is seeking compassionate nurse practitioners to perform consultations in nursing homes, assisted living facilities, and other settings. You will work with a team to deliver high-level care to older adults, with flexible schedules and strong clinical mentorship.

Qualifications include MSN with FNP/ANP/AGNP/etc, active SC license, DEA registration, and at least 1 year of NP experience.

Qualifications

• MSN with FNP/ANP/AGNP/AGACNP/AGPCNP/ACNP certification and DEA registration.
• Active South Carolina license to practice as NP.
• Minimum 1 year of nurse practitioner experience.

Responsibilities

• Round at assigned facilities to manage acute, subacute, and primary care needs of older adults.
• Collaborate with OnSite team, nursing home staff, residents, and families.
• Provide education on best practices and integrate non-pharmacological interventions when possible.
• Proactively follow up with patients and assess efficacy of interventions.
