UnitedHealth Group's Optum HouseCalls team is hiring a HouseCalls Advanced Practice Clinician to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap-closure, and collaboration with PCPs to improve health outcomes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted license to practice in the applicable state.
• Must have reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsabilités

• Conduct in-home assessments for Medicare Advantage members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close gaps in care.
• Identify urgent situations and intervene as needed.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
