Job summary

Optum Home & Community Care is hiring an Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage members. Flexible scheduling, a no on-call/no weekends/holidays policy, and a sign-on bonus support work-life balance in this field-based role.

Active state licensure and certification are required, along with a valid driver's license and reliable transportation for home visits.

Qualifications

• Active licensure in the practicing state with no disciplinary actions.
• NPs: ANCC or AANP Family/Adult/Geriatric cert; PAs: NCCPA cert and state licensure.
• Driver's license and reliable transportation for home visits.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plans.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close gaps in care.
• Identify urgent situations and intervene appropriately, as needed.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals when needed.
