Job summary

OneWell Health Care in Pennsylvania is seeking a dedicated Licensed Practical Nurse (LPN) to deliver skilled nursing services in home and community settings. You will work under the supervision of an RN Case Manager, implementing care plans, administering medications, and supporting individuals and families to maintain health, safety, dignity, and quality of life.

The role requires an active PA LPN license, CPR/First Aid, and at least one year of nursing experience, with preference for IDD/home

Qualifications

• Current PA LPN license
• CPR and First Aid certification required
• Additional certifications per waiver programs

Responsibilities

• Implement individualized care plans and administer prescribed meds.
• Perform clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring.
• Educate families and caregivers; participate in interdisciplinary meetings.
• Maintain accurate documentation in the EHR and ensure regulatory compliance.
