Job summary

Well Care Home Health, Inc. is seeking a Licensed Practical Nurse to support home health patients under the supervision of a registered nurse in North Carolina.

The role involves implementing the plan of care across life stages, from adults to geriatrics, in accordance with agency policies and nursing standards. You will participate in assessment, planning, implementation and evaluation, collaborating with a multidisciplinary team to ensure high-quality patient care and safety.

Qualifications

• Graduate of an accredited or approved school of practical nursing.
• Current license to practice practical nursing in North Carolina. CPR certification required.
• Minimum one year practical nursing experience; home health experience preferred.
• Hand/eye coordination to give injections, operate a computer, etc.; must be literate in English.
• Ability to develop positive interaction with patients, families, physicians and staff.

Responsibilities

• Assist with assessment, planning, implementation and evaluation based on patient age and diagnosis.
• Coordinate patient care with multidisiplinary team to ensure effective management.
• Communicate information per agency policies and discipline guidelines.
• Contribute to program effectiveness and efficiency in daily operations.
• Organize and perform work efficiently; maintain scheduling to optimize agency performance.
