Job summary

You will educate patients and families, update records, and participate in ongoing care planning, while adhering to agency policies and safety standards. A valid NC LPN license and CPR certification are required.

Qualifications

• Current license to practice practical nursing in North Carolina.
• CPR certification required.
• Minimum one year practical nursing experience; home health preferred.

Responsibilities

• Provide nursing care to patients in their homes, including medications and wound care.
• Monitor patients’ conditions and report changes to the care team.
• Educate patients and families on health management and self-care.
