Job summary

Well Care Home Health, Inc. is seeking a Licensed Practical Nurse to provide skilled in-home nursing under the supervision of a registered nurse.

The LPN will participate in the nursing process and implement the plan of care for adults and geriatric patients across life stages, in a home health setting. Ideal candidates hold an NC LPN license with CPR certification and at least one year of practical nursing experience, with home health experience preferred.

Qualifications

• Graduate of an accredited practical nursing program.
• Active NC LPN license; CPR certification required.
• Minimum 1 year practical nursing experience; home health preferred.

Responsibilities

• Assist with the assessment, planning, implementation and evaluation process per patient age and diagnosis.
• Coordinate patient care with multidisciplinary team to ensure effective management.
• Communicate information per agency policies and discipline guidelines.
• Contribute to program effectiveness and organize/work efficiently.
• Maintain schedules to support agency performance and patient care.
• Demonstrate agency values daily and maintain positive relationships with patients and staff.
• Promote patient/caregiver satisfaction and quality care across life stages.
