Job summary

Well Care Home Health of the Southern Triangle, Inc. seeks a licensed practical nurse (LPN) to join our home health team in North Carolina.

The role involves patient assessment, planning, implementation and evaluation within a multidisciplinary team, with emphasis on effective communication and adherence to agency policies. Qualified candidates hold an active NC LPN license, CPR certification, and at least one year of practical nursing experience (home health preferred).

Qualifications

• Graduate of an accredited or approved school of practical nursing.
• Current license to practice practical nursing in the State of North Carolina. CPR certification required.
• Minimum one year practical nursing experience required. Home health experience preferred.
• Hand/eye coordination to give injections and use computer; literate in English.
• Ability to transfer and guard patients; communicate effectively with patients, families, and staff.
• Physical requirements include lifting, repositioning and assisting patients; driving may be required.
• Ability to assess, plan, implement and evaluate patient care; prioritize duties.
• Ability to visually assess patients and read readouts; use equipment and read alarms.
• Exposure to hazards including bloodborne pathogens; work in patients' homes.
• Hours of Work: Variable Mon-Fri, weekends and holidays as needed.
• Population served: Adolescents, adults, geriatrics, pediatrics.
• Must have a valid North Carolina driver's license and an operational vehicle.
