Qualifications

• Minimum one year practical nursing experience; home health preferred.
• Ability to communicate effectively in English and operate necessary equipment.
• Ability to transfer and assist patients; strong interpersonal skills.
• Ability to assess patient conditions and participate in care planning.

Responsibilities

• Assist with assessment, planning, implementation and evaluation based on patient age/diagnosis.
• Coordinate patient care management with multidisciplinary team.
• Communicate information per agency policies and guidelines.
• Contribute to program effectiveness and agency mission.
• Organize and perform work efficiently; manage schedules.
• Assist in maintaining patient safety and comfort during home visits.
