Qualifications

• Graduate of an accredited or approved school of practical nursing.
• Current license to practice practical nursing in North Carolina; CPR certification required.
• Minimum one year practical nursing experience; home health experience preferred.
• Strong communication and literacy in English; team collaboration abilities.
• Excellent interpersonal interactions with patients, families, physicians and staff.
• Must have a valid North Carolina driver's license and reliable vehicle.

Responsibilities

• Assist with assessment, planning, implementation and evaluation based on patient age and diagnosis.
• Coordinate patient care management with multidisciplinary team.
• Communicate according to agency policies and guidelines.
• Promote program effectiveness and service quality.
• Maintain and adjust scheduling to support agency performance.
• Demonstrate agency values and mission daily.
