Job summary

Well Care Health seeks a compassionate Licensed Practical Nurse for in-home patient care. You will provide medications administration, wound care, and other treatments in patients' homes under RN supervision.

Responsibilities

• Assist with assessment, planning, implementation and evaluation per patient age/diagnosis.
• Coordinate patient care with multidisciplinary team.
• Communicate information per agency policies and guidelines.
• Contribute to program effectiveness and maintain efficient workflow.
• Adjust schedules to enhance agency performance.

• Provide patient education and at-home nursing care.
