Job summary

Well Care Home Health of the Southern Triangle, Inc. seeks an LPN to assist with patient assessment, planning, implementation and evaluation in home health settings across adolescents to geriatrics.

You will coordinate care with a multidisciplinary team, communicate per agency policies, and contribute to program effectiveness while maintaining schedules and promoting patient satisfaction. The role requires NC practical nursing licensure, CPR certification, and at least one year of nursing

Qualifications

• Education: Graduate of an accredited or approved school of practical nursing.
• Licensure: Current license to practice practical nursing in the state (North Carolina).
• Experience: Minimum one year practical nursing experience; home health experience preferred.
• Essential Technical / Motor Skills: Hand/eye coordination for injections, computer use; English literacy; can operate patient care equipment.
• Interpersonal Skills: Ability to develop positive interaction with patients, families, physicians and staff.
• Essential Physical Requirements: Ability to transfer and move patients weighing at least 50 pounds; frequent lifting and ambulation.
• Essential Mental Abilities: Assess patient condition, plan and evaluate care, adapt to changes, memory and learning.
• Essential Sensory Requirements: Visual and auditory assessment, reading readouts, listening for alarms; effective communication.
• Exposure to Hazards: Possible exposure to bloodborne pathogens and safety risks in home settings.
• Hours of Work: Variable Mon-Fri, weekends and holidays as needed.
• Population Served: Adolescents, adults, geriatrics, pediatrics.
• Must have a valid NC driver's license and an operational vehicle.
