Elara Caring’s Home Health team in Massachusetts is hiring a Licensed Practical Nurse (LPN) for full-time, Monday–Friday, 8 a.m.–5 p.m., visiting patients in their homes.

You will deliver skilled nursing care per care plans, monitor progress, educate patients and families, document visits, and coordinate with an RN Case Manager. A MA LPN license and at least one year of experience are required, with a valid driver's license and reliable transportation.

Qualifications

• Massachusetts LPN license and graduation from an accredited nursing program.
• At least one year of clinical nursing experience.
• Valid driver's license with reliable transportation and auto insurance.

Responsibilities

• Provide skilled nursing care according to each patient’s individualized care plan and physician orders.
• Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
• Teach patients and caregivers techniques that support health, safety, and independence at home.
• Collaborate with the RN Case Manager as patient needs and care plans change.
• Document visits, nursing interventions, and medication information accurately and promptly.
• Communicate with physicians, patients, families, and the clinical team to keep care coordinated.
• Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.
