Trinity Health At Home’s Holy Cross Home Care and Hospice in Maryland is seeking a Home Health Licensed Practical Nurse (LPN). You will deliver skilled nursing care to clients per the physician-approved plan of care, assigned by the supervising RN, in a home setting.

Qualifications

• Must have a minimum of one (1) year experience in an acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing
• Current licensure in the State of Maryland
• CPR certification required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
• Must have current Driver’s license and reliable transportation to and from work site.

Responsibilities

• Deliver skilled nursing care to clients per the plan of care prescribed by the physician and assigned by the supervising RN.
• Provide care in clients' homes with autonomy and 1:1 patient focus.
• Maintain documentation and care coordination using the Epic EHR system.
• Collaborate with the nurse-led leadership and care team to ensure quality outcomes.
• Practice at the top of your license and advocate for patients as needed.

Minimum Qualifications
