Holy Cross Home Care and Hospice (Maryland) is seeking a Home Health Licensed Practical Nurse (LPN) to deliver skilled nursing care in Montgomery County. You will implement the physician’s plan of care and collaborate with a supervising RN to ensure high-quality, patient-centered service.

The role emphasizes 1:1 care, autonomy in the home setting, and opportunities for professional growth. We offer a comprehensive benefits package starting Day 1 and a supportive nurse-led leadership team.

Qualifications

• Minimum of one year experience in an acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing.
• Current licensure in the State of Maryland.
• CPR certification required.
• Must have passed NAPNES or equivalent medication exam before administering medications.
• Must have a valid driver’s license and reliable transportation to the work site.

Responsibilities

• Deliver skilled nursing care per the care plan prescribed by the physician and as assigned by the supervising RN.
• Provide 1:1 patient care with a focus on independence and safety in the home.
• Document care and coordinate with the team using the Epic EHR system.

Minimum Qualifications

• Must have a minimum of one (1) year experience in an acute care setting.

• Home healthcare experience preferred.
