Holy Cross Home Care and Hospice (Maryland) is seeking a Home Health LPN to deliver skilled nursing care at patients' homes under physician plans and RN supervision. The role supports top-of-license practice with autonomy, meaningful 1:1 patient relationships, and flexibility across Montgomery County.

Qualifications

• Minimum 1 year experience in acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing.
• Current Maryland LPN license.
• CPR certification required.
• Must have passed NAPNES or equivalent medication exam before administering medications.
• Must have valid driver’s license and reliable transportation to work site.

Responsibilities

Minimum Qualifications

• Must have a minimum of one (1) year experience in an acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing
• Current licensure in the State of Maryland
• CPR certification required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
• Must have current Driver’s license and reliable transportation to and from work site.
