Job summary

Trinity Health At Home – Muskegon is seeking a Home Health Physical Therapist to deliver one-on-one therapy in patients' homes, assessing needs and developing personalized treatment plans. You will provide evidence-based interventions, educate patients and caregivers, and coordinate care with a patient-centered approach to promote independence and safe recovery at home.

Qualifications

• Current licensure as a Physical Therapist in Michigan.
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience; home care preferred.
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.

Responsibilities

• Provide skilled, one-on-one therapy services in patients’ homes.
• Assess patient needs and develop individualized treatment plans.
• Deliver evidence-based interventions.
• Educate patients and caregivers to support safe recovery at home.

As a Home Health Physical Therapist, you’ll provide skilled, one‑on‑one therapy services in the comfort of patients’ homes, helping them regain strength, mobility and independence.
