The successful candidate will have a nursing or physical therapy license, 1 year of Medicare-certified home health experience with Start of Care OASIS

Qualifications

• RN or PT with active license in Arizona or compact state
• 1 year Medicare-certified home health experience including Start of Care OASIS competence
• 2 years RN or PT experience; supervisory or lead experience preferred

Responsabilités

• Oversee coordination of patient plans of care delivered by home health staff.
• Ensure services meet quality, safety, and regulatory standards.
