HonorHealth is seeking an Occupational Therapist II for Home Health in the Deer Valley/ Phoenix area. The role focuses on evaluation, treatment planning, and oversight of evidence-based therapy services for individuals with physical disabilities to improve daily living skills.

The position requires an OT license, BLS certification, and a valid driver's license. Typically involves collaborating with physicians, family members, and other healthcare professionals to achieve optimal patient outcomes

Qualifications

• Bachelors in in Occupational Therapy from an accredited university (Required)
• Masters in in Occupational Therapy from an accredited university approved by the AOTA Preferred
• 1 year in Home Health experience or internship (Preferred)
• 3 years in related experience (Preferred)
• Basic Life Support (BLS) - Certification (Required)
• Drivers License (DL) - License (Required)
• Occupational Therapist (OTL) - License (Required)
• Automobile Insurance (Required)

Responsibilities

• Communicates and documents communications with attending physicians, other disciplines and case managers regarding patient condition and discharge planning.
• Gathers data related to patient condition/history, selects evaluation tools, and administers evaluations to determine functional abilities.
• Implements an individualized treatment plan using therapeutic exercises, manual techniques, mobility training, and ADL training.
• Educates patients, families and providers to enable cost-effective health care decisions.
• Participates in quality assurance, improvement programs, and audits.
• Establishes treatment plans for Home Health Aides and monitors care plans.
