Job summary

HonorHealth seeks a Home Health Supervisor to lead clinical and non-clinical staff, oversee patient plans of care, and ensure quality and regulatory compliance within our Deer Valley home health team in Phoenix, AZ.

The role requires an active RN or PT license with Medicare home health experience; leadership background and Start of Care OASIS competence are preferred. Office hours are 8 am–5 pm, with on-call after hours as needed.

Qualifications

• RN or PT license and active licensure in AZ or compact system.
• Start of Care OASIS assessment competence required.
• Minimum 1–2 years RN or PT skilled Medicare-certified home health experience; supervisory/leadership preferred.

Responsibilities

• Oversee coordination, implementation, and evaluation of patient plans of care.
• Supervise clinical and non-clinical staff; manage scheduling, workflow, and documentation compliance.
• Lead process improvements and ensure CMS/state payer/HonorHealth requirements are met.
• Provide after-hours administrative support as part of on-call duties.
