Job summary

Yale New Haven Health is seeking a Call Center/referral information specialist in Connecticut. The role involves receiving referrals from various sources, interpreting reimbursement resources, and utilizing agency systems to support patient access to services.

The position requires RN or BSN (preferred) with CT licensure and at least 1 year of community health nursing experience, plus strong telephone and computer skills.

Qualifications

• B.S.N. (preferred) or RN with CT license, plus one year recent experience in community health nursing.
• Strong telephone and computer skills; satisfactory work references.
• Sedentary work mainly; may involve walking/standing briefly; able to handle up to 50 pounds occasionally.

Responsibilities

• Receives referrals from all sources.
• Interprets reimbursement information and available resources for callers.
• Uses agency information systems as appropriate.
• Requests RN, therapist, and home health aides as needed.
• Coordinates and collaborates patient care with physicians, other agencies, providers and community organizations.
• Participates in at least 12 hours of education per year.
• Performs other duties as assigned by Call Center Supervisor and Director of Clinical Services.
• Proj ects a positive attitude about the agency mission, philosophy, and policies.
