Job summary

Pathways Healthcare in Worcester, MA is seeking a dedicated Registered Nurse to join our in-home health and hospice team. You will assess patients, develop and revise plans of care with families, administer treatments, and collaborate with physicians to deliver high-quality, compassionate care.

You will work weekdays with a focus on patient-centered outcomes, driving improvements in home-based rehabilitation and palliative nursing care while maintaining strict compliance with healthcare

Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient’s status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Develops a care plan based on nursing diagnosis with patient and family involvement.
• Administers medications and treatments as prescribed by the physician.
• Completes and maintains clinical notes and tracks pain/symptom changes.
• Communicates with the physician regarding patient needs and changes in condition.
