Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.

Responsibilities

• Assess and evaluate patient status; write and initiate plan of care.
• Regularly re-evaluate patient and caregiver needs; revise plan as needed.
• Develop care plans with goals based on nursing diagnosis and involve patient and family.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical notes and report changes to physician.
• Ensure patient safety and participate in interdisciplinary meetings.
