Job summary

Pathways Healthcare, Llc is seeking a compassionate Hospice Registered Nurse to join our growing team. The role involves in-home hospice care across Fall River and surrounding areas, with a Monday–Friday schedule and a supportive environment.

We value bedside care excellence, strong communication, and adherence to care plans. A degree in nursing, 2+ years of nursing experience, and current BLS/CPR certification are required, along with a valid driver’s license.

Qualifications

• 2+ years nursing experience, preferably in geriatrics
• Degree in Nursing from an accredited institution
• Proficient with electronic medical records
• Excellent bedside manner and communication skills
• Working knowledge of health care laws and regulations
• Maintain current BLS CPR certification

Responsibilities

• Assesses and evaluates patient’s status and writes plan of care
• Regularly re-evaluates patient and family/caregiver needs
• Develops and implements a patient-centered care plan
• Administers medications and treatments as prescribed
• Maintains accurate clinical notes and tracks pain/symptom changes
• Communicates with physician and obtains orders as required
• Creates a safe environment and participates in interdisciplinary meetings
• Participates in on-call duties
• Ensures equipment and services are available
• Engages in continuing education and professional development

At Pathways Healthcare, we are thrilled to embark on our new journey into Hospice care. As we expand our services, we are looking for a compassionate and skilled Hospice Registered Nurse to join our team.

• Main Office location: Westwood, MA
• Service Area: Fall River and Surrounding Areas
• Job Type: Full-time
• Schedule: Monday – Friday

Responsibilities:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.

• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
