Job summary

Pathways Healthcare in Massachusetts is seeking an experienced registered nurse to join our home health and hospice team. This full-time role offers a Monday–Friday schedule and the opportunity to impact patients by delivering care at home.

You will assess patients, administer treatments, develop care plans with physicians, and coordinate with interdisciplinary teams. A valid MA nursing license, 2+ years in home health/geriatrics, and a strong patient focus are required.

Qualifications

Active Nursing licensure in Massachusetts.
Preferably 2+ years of nursing experience, home health/geriatrics.
Nursing degree from an accredited institution.
Excellent bedside manners and communication skills.
Working knowledge of healthcare laws and regulations.
Current BLS CPR certification.
Valid driver’s license and own vehicle.

Responsibilities

Initiates preventive and rehabilitative nursing procedures.
Administer medications and treatments as prescribed by physicians.
Conduct initial assessments of patients and families to determine home care needs.
Perform thorough physical assessments and gather medical histories.
Develop and revise individualized plans of care based on patient status.
Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create plans of care.
Address inquiries and concerns from patients, caregivers, and medical teams.
Provide counseling and support to patients and caregivers before, during, and after treatment.
Supervise home health aides as required.
