Pathways Healthcare in Boston, MA is seeking a Registered Nurse or Licensed Practical Nurse to join our home health and hospice team. This full-time role offers a Monday-Friday schedule and the opportunity to impact patients' lives by delivering in-home care with dignity.

You will assess patients, write and revise plans of care, administer medications, collaborate with physicians and participate in quality improvement.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient’s status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.
• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
