Job summary
Pathways Healthcare in Worcester, MA is seeking a dedicated Nurse Practitioner to join our physician- and NP-led home health and hospice team. You will conduct admissions, create treatment plans, and provide routine and urgent home visits.

Qualifications
- APRN license with DEA, CPR certification.
- Master’s degree in Advanced Practice Nursing.
- Minimum 2 years of home health experience.
- Valid driver’s license.
Responsibilities
- Conduct admission assessments and develop personalized treatment plans.
- Provide routine and urgent home visits.
- Manage medication orders and educate patients on proper usage.
- Handle wound care, testing, and equipment needs.
- Document patient progress and coordinate with the care team.
- Assist in discharge planning and ensure confidentiality.
