Pathways Healthcare in Massachusetts is seeking a skilled Registered Nurse to join our home health and hospice team. You will assess patients, write and adjust plans of care, and collaborate with physicians to deliver compassionate, patient-centered care.

This full-time role offers day shifts, on-site visits, and opportunities for leadership development as we expand in Beverly, Danvers, Lynn, Peabody and surrounding towns.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient’s status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures including administering medications and treatments as prescribed by the physician.
• Completes, maintains and submits accurate clinical notes and records pain/symptom changes.
• Communicates with the physician regarding patient needs and reports changes in condition.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.
• Participates in on-call duties.
• Supervises ancillary personnel and delegates responsibilities when required.
• Maintains professional knowledge through continuing education.
