Job summary

Pathways Healthcare seeks a Home Health + Hospice Aide to deliver compassionate, patient-centered care in home settings. You will work under RN supervision, observe patients, document care, and contribute to a rehabilitation-focused approach that respects patient dignity.

This full-time position offers a Monday–Friday schedule (8:30 a.m.–5:00 p.m.) in Waltham and surrounding areas, with a competitive $25/hour pay and a comprehensive benefit package designed to support clinicians and patients

Qualifications

• MA CNA preferred; HHA certificate accepted.
• Must be at least 18 years old.
• Ability to read and follow written instructions and document care.
• Self-directed with minimal supervision.
• Empathy for the needs of the ill and impaired.
• CPR certification maintained.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Observe patients, report findings and document care performed.
• Work under a Registered Nurse to promote quality, continuity and safety of care.

Job Description

• Role: Home Health + Hospice Aide


• Rate: $25/hour


• Service Area: Waltham and surrounding areas


• Schedule: Monday-Friday (8:30-5:00pm)


• Job Type: Full-Time

Qualifications

• Certified Nursing Assistant in Massachusetts (preferred); Home Health Aide certificate accepted


• At least 18 years of age


• Ability to read and follow written instructions and document care given


• Self-directing with the ability to work with little direct supervision


• Empathy for the needs of the ill, injured, frail and the impaired


• Possess and maintains current CPR certification


• Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order

Responsibilities

The Home Health Aide is a Certified Nursing Assistant and paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The HHA is responsible for observing patients, reporting these observations and documenting observations and care performed. The HHA will be assigned by a Registered Nurse in a manner that promotes quality, continuity and safety of a patient’s care.
