Pathways Healthcare in Westwood, MA is hiring a registered nurse for our home health and hospice team. The role focuses on in-home nursing care, medication administration, and plan-of-care development, with emphasis on geriatrics.

We leverage AI-assisted documentation to save clinicians time while maintaining clinical judgement at the center of each note. This is a full-time position with competitive benefits.

Qualificações

• Nursing degree from an accredited institution.
• Preferably 2+ years of nursing experience with geriatrics.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsabilidades

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and revises as patient status changes.
• Responds promptly to afterhours calls (Weekends 8:30 AM to 8:00 PM).
• Assesses patient needs through triaging and in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.

Job Description:

• Salary: Commensurate with Experience

• Service Area: Cranston, Fall River and surrounding territories

• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
