Pathways Healthcare in Massachusetts seeks a Registered Nurse - Home Health to serve Methuen and surrounding areas. Typical schedule is Monday–Friday, 8:30 AM–5:00 PM, full-time.

The candidate should have 2+ years of nursing experience, preferably in geriatrics, and a nursing degree from an accredited institution. We value EMR proficiency, strong bedside manner, knowledge of health care laws, a valid driver's license, and access to a reliable insured vehicle.

Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• New nurses are welcome; great training program available.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient’s status by writing and initiating plan of care.
• Regularly re-evaluates patient and family/caregiver needs.
• Participates in revising the plan of care as necessary.
• Develops a care plan with goals based on nursing diagnosis including family involvement.
• Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
• Completes and submits accurate clinical notes; monitors pain/symptom changes.
• Communicates with physician regarding patient needs and requests orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities
