Job summary

Pathways Healthcare seeks a Hospice Home Health Aide to support patients in Worcester and surrounding areas. Part-time, Monday–Friday 8:30–5:00 with no nights or weekends. Responsibilities include personal care, documentation, following RN care plans, and maintaining a safe home environment.

Must be 18+, CPR, MA-licensed CNA preferred or HHA certification accepted. Driving required with reliable insured vehicle. Mileage reimbursement offered.

Qualifications

• Certified Nursing Assistant (CNA) in Massachusetts preferred; Home Health Aide certification accepted
• Must be 18 years of age or older
• Ability to read and follow written care plans and document services provided
• Ability to work independently with minimal supervision
• Compassion and empathy for individuals who are ill, injured, or elderly
• Current CPR certification required
• Strong interpersonal skills, patience, and professionalism
• Valid driver’s license and reliable insured vehicle
• Ability to assist patients with repositioning, transferring, and other physical tasks

Responsibilities

• Provide personal care assistance such as bathing, grooming, dressing, and mobility support
• Observe and report changes in patients’ condition to the supervising nurse or therapist
• Document care provided and patient observations accurately
• Follow the care plan established by the Registered Nurse
• Maintain a safe and supportive environment for patients in their homes
• Promote quality, continuity, and safety of patient care

• Service Area: Worcester and Surrounding Areas
