Job summary

Choice Health At Home in Conroe, TX seeks a Home Health Business Development Liaison to grow referrals with clinicians, hospitals and community organizations. You will promote our home health services and align offerings with market needs.

You will collaborate with clinical and administrative teams to ensure compliant outreach, support program development, and drive quarterly admission goals in a performance-driven environment.

Qualifications

• Minimum of 2 years home health or hospice sales experience preferred.
• CRM Software experience preferred.
• Valid Driver’s License.
• Valid Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Establish new referral relationships, presenting Choice Health At Home’s care model to become the provider of choice.
• Counsel the Agency on community needs; develop programs including personnel and financial needs.
• Develop and maintain policies for new programs.
• Ensure compliance with local, state and federal marketing laws.
• Collaborate with administrative and supervisory personnel to direct programs.
• Partner with the Interdisciplinary Group to support safe patient/family care.
• Establish a public relations program to interpret the agency’s services and foster relations with physicians and community agencies.
• Meet and/or exceed admission goals monthly.
