Job summary

GrandCare Health Services in Southern California is seeking a Community Liaison – Home Health to establish and maintain positive relationships with physicians, discharge planners, case managers and other health care professionals who utilize home health services.

You will respond to customer requests, negotiate service contracts with payers, and lead marketing initiatives to grow referrals and patient volume while upholding GrandCare’s commitment to quality care.

Qualifications

• Bachelor’s degree in Marketing, Business Administration, or related field preferred, but not required.
• Health care marketing management in home health care operations.
• Ability to market aggressively and deal tactfully with customers and the community.
• Knowledge of corporate business management.
• Demonstrates good communications skills, negotiation skills, and public relations skills.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Responsibilities

• Develop and maintain relationships with physicians, discharge planners, case managers, social workers, and other health care professionals.
• Schedule and conduct visits to referral sources to identify patients suitable for home health services.
• Maintain knowledge of agency/market trends, coverage criteria, and regulations.
• Represent the agency at health fairs and community events; participate in civic committees.
• Develops market assessments and marketing plans to meet volume projections.
• Establishes and maintains relationships with referral and payer sources; monitors perceptions of GrandCare as a high quality provider.
