Job summary

Enhabit Home Health & Hospice is seeking a candidate to serve as the primary liaison between the company and referral networks. You will educate referral sources on services and navigate post-acute care transitions to boost patient outcomes and satisfaction.

The role requires healthcare business development experience, strong communication skills, and knowledge of Medicare/Home Health regulations. In-office presence is typical for collaboration and supervision.

Qualifications

• Bachelor's degree or graduate of approved nursing/therapy/social work program or equivalent experience.
• Minimum two years in healthcare business development, community relations, marketing or sales.
• Knowledge of Medicare home health and hospice regulations.

Responsibilities

• Acts as primary relationship manager with referral sources including physicians and hospitals.
• Promotes awareness of services and program requirements to referral sources and families.
• Supports post-acute care transitions to improve patient outcomes and satisfaction.
