Job summary

AHN Healthcare@Home is seeking a Home Health CNA to provide compassionate, in-home care in the East Liberty area of Pittsburgh, PA. This 40-hour Employee role involves ADLs, clinical observation, and EMR documentation as part of an interdisciplinary team.

You will travel to patient homes, work independently yet rely on team support, and contribute to comfort and quality of life for patients in their homes.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience.
• A valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands‑on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
