As a CNA, you will assist with ADLs, observe changes, document in EMR, and collaborate with the care team to ensure comfort and safety. A valid driver’s license is required for daily travel within the territory.

Qualifications

• Active CNA certificate in the state you work.
• High School diploma or certificate.
• Minimum one year healthcare experience.
• Valid driver’s license and ~20% travel to patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a home health setting.
• Assist with activities of daily living (ADLs) and observe changes.
• Educate and support patients and caregivers on care plans.
• Respond to patient needs and document in the EMR.
• Work independently, but collaborate with the care team.
• Travel to patient homes within assigned territory.
