AHN Healthcare@Home in East Liberty, PA is hiring a Home Health CNA to provide compassionate care in patients' homes within the assigned territory. You will work 40 hours per week as an employee, collaborating with an interdisciplinary team to ensure comfort, dignity, and quality of life for patients.

Requirements include an active CNA certificate, High School Diploma, and a valid driver's license with approximately 20% travel.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Valid driver’s license and 20% travel to patient locations required

Responsabilités

• Deliver compassionate, patient-centered care in home/hospice settings with an interdisciplinary team
• Assist with activities of daily living (ADLs) and observe/report changes in condition
• Educate and support patients and caregivers on care plans and routines
• Maintain accurate, timely documentation in the EMR
• Work independently while collaborating with the care team
• Travel to patient homes within assigned territory
