AHN Healthcare@Home in East Liberty, PA is hiring a Home Health CNA to provide compassionate care in patients' homes within the assigned territory. You will work 40 hours per week as an employee, collaborating with an interdisciplinary team to ensure comfort, dignity, and quality of life for patients.

Requirements include an active CNA certificate, High School Diploma, and a valid driver's license with approximately 20% travel.

Qualificações

Responsabilidades

We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the East Liberty area.

What You'll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
