Job summary

AHN Healthcare@Home in Pittsburgh is seeking a Home Health LPN to join our patient‑focused team. You will collaborate with the care team to manage and deliver patient care plans, and you will travel to patient locations to provide direct care while documenting care using EMR software.

Qualifications

LPN license in the state where you work.
Graduate from an approved school of practical nursing.
Two years of LPN experience.
Current driver’s license and ability to travel to/from patient locations.
Proven ability to self‑manage time and schedule.
Experience with tablets, mobile phones, and EMR software.

Responsibilities

Collaborate with the care team to manage and deliver patient care plans.
Maintain accurate charting and meet critical deadlines for documentation.
Provide direct patient care while traveling to patient locations; approximately 20% of your day will be spent driving.
