Job summary

Enhabit Home Health & Hospice in Grapevine, TX is seeking a registered nurse to coordinate comprehensive home health services. You will review patient information, support care planning, and facilitate collaboration among physicians, patients, caregivers, and field staff.

Ideal candidate has RN licensure, CPR, at least two years of nursing experience (including home health), valid driver’s license, and proficiency with mobile devices.

Qualifications

• Graduate of an approved school of professional nursing.
• Current valid nursing license; CPR certification.
• Minimum two years of nursing experience; home health experience preferred.
• Valid state driver’s license and automobile liability insurance.
• Basic technology skills; capable with mobile devices.
• Preferred bilingual English/Spanish.

Responsibilities

• Coordinate and oversee home health services per standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Liaise among physicians, referral sources, patients, caregivers and field staff to ensure coordinated care.
• Schedule patients to field clinicians and communicate with all stakeholders to maintain continuity of care.
